Roadmap: From Scribe to Health Research Company

The four stages, the prevention features to come, and how 1Heath becomes a research and development company without ever selling patient data.

Key Points
  • Four stages: a safe scribe for one clinic; one record across doctor, desk, pharmacy and patient; checks that prevent harm; and, with consent and ethics approval, research that makes Indian medicine safer.
  • Each stage starts only when the one before is measured on real patients, not when it is built.
  • The long-term aim is a health research and development company built on India's own clinical data: Indian languages, every system of medicine, and Indian patients.
  • Patient data is never sold. Research uses only consented, de-identified data, under ethics review, with results published.

The Four Stages

  1. 1ScribeThe doctor talks; a verified note and prescription are signed
  2. 2Shared recordDesk, pharmacy, patient and family on one consented record
  3. 3PreventionChecks that catch harm before it happens
  4. 4ResearchConsented data that answers questions no one in India can answer today
Figure 1. Each stage depends on the one before: no prevention without a record, no research without trustworthy prevention data.

Stage 1 and 2: Scribe and Shared Record

FeatureWhat it doesGate before release
Doctor app pilotListen, draft, verify, sign, prescription PDFConsent at the desk; legal and ethics review; zero invented medicines on real voices
Verified doctorsDoctors verified against India's national registry of healthcare professionals[1]Registry integration tested
Desk appRegistration, ABHA link, consent, vitals, weight for childrenConsent text reviewed by counsel
Pharmacy appReceives the signed prescription and allergy flags; records what was dispensedPharmacist field test
Patient and family appVisit summaries in the patient's language, reminders, family record, emergency cardGuardian consent for children's records[2]
ABDM exchangeShare records with other providers through ABDM with the patient's consentABDM sandbox certification

Stage 3: Prevention Features

FeatureThe harm it targets
Medicine list across all systemsDuplicates and interactions between modern, Ayurvedic and homeopathic medicines the doctor never hears about
Weight-based dose checks for childrenDosing errors, the most common cause of potentially harmful errors in children[3]
Pregnancy checksMedicines unsafe in pregnancy given to a woman who may be pregnant
Antibiotic stewardship promptsOveruse of WHO "Watch" antibiotics, over half of doses in India[4]
Banned-combination checksIrrational fixed-dose combinations
Chronic-care follow-upMissed refills and checks for diabetes and high blood pressure
Family screening promptsRelatives of a patient with an inherited condition never offered testing (with consent and counselling)
3D scan viewerPatients who cannot understand their own CT or MRI (from the imaging project on the home page)

Checks that influence a clinical decision may make 1Heath regulated medical device software. Each one will be assessed against CDSCO's guidance before release[5].

Stage 4: A Health Research Company

About half of Indians use AYUSH or home remedies[6], yet no record anywhere holds what one patient takes from every system at once, so questions about combined use and harm are hard to study. A signed, structured, consented record across every system of medicine makes them answerable.

Research programmeQuestion it answersPartner type
Medicine safety signalsWhich combinations, including Ayurvedic and modern medicines, are followed by harm?National pharmacovigilance programmes, including the AYUSH programme[7]
Indian clinical languageHow do Indian doctors and patients describe illness and medicines across languages?Universities, speech and language researchers
Antibiotic useWhere and why are broad-spectrum antibiotics prescribed, and does prompting change it?Public-health researchers; India has no formal surveillance of antibiotic use[4]
Real-world outcomesHow do Indian patients respond to common treatments outside trials?Medical colleges, government research programmes such as PRIP[8]
Trial recruitmentCan patients who opt in be told about trials they qualify for?Hospitals and sponsors, with ethics approval

Research Rules

  • 1

Separate, explicit consent for research, never bundled with care. Refusing changes nothing about the patient's care.

  • 2

De-identified data in a controlled environment; no raw records leave it.

  • 3

Independent ethics review of every study, and a data-access committee that includes doctors and patients.

  • 4

    Results published, including negative ones.

  • 5

Never sold. Partners fund studies; they never buy data.

Our Own Models

Speech and drafting are 1Heath's largest cost per consultation (see the business model). Training India-hosted speech and drafting models on consented, de-identified consultations would cut that cost, keep data in India, and improve accuracy on code-mixed Indian speech. This begins only after the consent and research rules above are in force.
